Job summary

UF Health Central Florida is seeking a Case Manager to utilize professional nursing skills to assess patient and family scenarios for clinical appropriateness for the setting and to establish plans for effective management of the continuum of care.

Assessment, planning, intervention and reassessment occur in a collaborative environment to optimize quality of care, utilization, cost and patient satisfaction.

Qualifications

• Minimum of 5 years experience; case management experience preferred.
• CCM certification preferred.
• Must read, write, speak and understand English.

Responsibilities

• The Case Manager assesses patient and family scenarios for clinical appropriateness and establishes plans for the continuum of care.
• Effective communication with physicians, patients/clients, families and staff to facilitate coordination of care.
