Job summary

Accord is hiring a Case Manager to support clients in Dakota, Hennepin, and Washington counties. This hybrid role blends office work in St. Paul with community visits, offering $23-$25 per hour and a $1500 new hire bonus paid over the first year.

You will assess needs, develop care plans, coordinate services, and monitor progress in the community. Qualified candidates hold a Bachelor's in Human Services or related field, or are licensed Social Workers; others with relevant experience may apply.

Qualifications

• Bachelor's degree in Human Services or related field or licensure as a Social Worker.
• If not in Social Work, at least 1 year of experience with mental illness or disability.
• Excellent written and verbal communication; able to work independently.

Responsibilities

• Develop community support goals and plans for individuals (typical caseload 40-45).
• Assist access to services and document case notes with service agreements.
• Inform guardians of suitable service options and options that fit needs.
• Meet with individuals regularly to evaluate progress and adjust plans.
