Job summary

St. Anne Institute is seeking a Case Manager for the Crisis Residence – "The Beacon". You will coordinate treatment plans and liaise with families and the community to support youth in crisis.

The role requires regular contact with youth, families and providers, with some evenings and weekends. A Bachelor's degree with two years in mental health or case management is preferred; MA/MSW is a plus. Eligible benefits include medical, dental, vision, and 403(b).

Qualifications

• Bachelor's degree with 2 years experience in mental health or case management preferred
• MA or MS in Human Services or MSW preferred
• Valid driver's license preferred

Responsibilities

• Maintain regular contact with youth, family and outside providers
• Provide non-clinical and outreach services based on needs assessment
• Accompany youth to medical/confidential appointments as necessary
• Maintain updated progress notes in EHR for assigned youth and family
• Send out referral packets and assist in discharge planning
• Assist other case managers with transportation, scheduling, visitation
• Attend Crisis Residence/Clinical staff meetings
• Attend mandatory in-services and trauma-informed trainings

Crisis Residence – “The Beacon”

Salary range:

$44K-54K

40 hours per week; includes some evenings and weekends

Job Description:

Under the direction of the Program Administrator, the case manager coordinates implementation of treatment plans and acts as a liaison between St. Anne Institute, families and the community. The case manager works to achieve change in the youth by influencing the “external” environment such as in the Crisis Residence Program, school and family and works to support the direction taken by clinical therapist who effects changes “internally” in the youth’s thinking, feelings and behavioral choices.

Responsibilities:

• Maintain regular contact with assigned youth, family and outside providers
• Provide non-clinical and outreach services based on needs assessment of caseload, including transportation, visitation, assistance with daily living skills, safety planning and additional services as needed
• Accompany youth to medical/confidential appointments as necessary
• Maintain updated progress notes of significant case events, treatment or special meetings and all contacts for assigned youth, family and service providers in agency’s Electronic Health Record system
• Send out referral packets and assist in discharge planning
• Assist other case managers when necessary (transportation, scheduling, visitation)
• Attend Crisis Residence/Clinical staff meetings
• Attend all mandatory in-services and outside workshops as required by the agency plus additional trainings in evidence-based and trauma-informed care

Requirements and Education:

• Empathetic and an ability to use creative problem solving; strong verbal and written communication skills; Bachelors degree with two (2) years experience in mental health or case management preferred; MA OR MS in Human Services OR MSW preferred; valid driver’s license preferred.
