Job summary

Family & Children's Center is seeking a compassionate CSP Case Manager to support individuals with severe mental illness in Community Support Program services.

You will assess needs, develop recovery plans, coordinate with providers, and connect clients to housing, healthcare, and community resources, while guiding their independence and stability in the community.

Qualifications

• Bachelor's degree in behavioral science or related field plus 1,000 hours of supervised post-degree clinical experience with persons with chronic mental illness; OR
• Bachelor's degree in another field plus 2,000 hours of supervised post-degree clinical experience with persons with chronic mental illness.

Responsibilities

• Provide mental health case management services to individuals with severe and persistent mental illness.
• Complete assessments and create treatment, recovery, and crisis plans.
• Help clients identify and achieve their individual recovery goals.
• Provide strengths-based, recovery-oriented services.
• Help clients manage psychiatric symptoms and co-occurring medical and substance use concerns.
• Provide symptom monitoring, education, crisis intervention, and medication education/monitoring as directed.
• Connect clients with community resources related to housing, food, healthcare, safety, and independent living.
• Coordinate care with internal and external treatment providers.
• Engage family members and support systems when permitted by the client.
• Participate in treatment meetings and ongoing supervision.
• Maintain accurate, timely clinical and case management documentation.
• Provide after-hours crisis services by phone or face-to-face when needed.
• Assist with discharge planning and referrals.

What You’ll Do

As a CSP Case Manager, you will:

This is a community-based position, with at least 50% of the work occurring in community settings or clients' homes. Local travel is required.

Required Qualifications

Education & Experience:

• Bachelor's degree in behavioral science or a related field plus 1,000 hours of supervised post-degree clinical experience with persons with chronic mental illness; OR
• Bachelor's degree in another field plus 2,000 hours of supervised post-degree clinical experience with persons with chronic mental illness.

Candidates should also have familiarity with the human service delivery system, DHS 63, recovery principles, ethics, and professional boundaries.

A valid driver's license and ability to use your own vehicle for client transportation are required.

Preferred Qualifications

• Master's degree in Social Work, Clinical Psychology, or Psychiatric Mental Health Nursing
• Additional supervised clinical experience working with individuals with severe and persistent mental illness
• Previous Community Support Program (CSP) experience
