Job summary

Family & Children’s Services in Oklahoma City seeks a dedicated Behavioral Health Case Manager II to coordinate services, advocate for clients, and help families access community resources. You will develop individualized recovery plans, document progress, and collaborate with a multidisciplinary team.

Your role requires trauma-informed care, strong communication, and compliance with privacy and funding regulations, with occasional evening or weekend hours and driving to community sites.

Qualifications

  • ODMHSAS/HOPE Behavioral Health Case Manager II certification required or pursued.
  • Case management experience in behavioral health or social services preferred.
  • Ability to complete thorough clinical documentation and reporting.

Responsibilities

Job description

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Coordinates the needs of clients and family members with resources available and provides client advocacy as needed.

Assistsclients in finding and accessing community resources needed tomaintainindependent functioning.

Assistsclients, individually and in groups, with development of independent and community living skills, social skills, interests, and leisure time activities which will encourage community involvement and integration.

Assistclients in developing an individualized recovery plan that focuses on personal and interactive problems, independent living skill needs, defining goals, andto planaction reflecting interests, abilities, and needs.

Establishes rapport with clients, assessing client needs; organizes,plans,and delivers face-to-face client services; and conductsfollow upon the outcome of services provided.

May provide services through individual or group rehabilitation to address symptoms through use of curriculum-based education and skills practice toassistclients in developing skills necessary to perform daily living and successful integration into community life.

Adheres to agency policy and procedures, as found in the Forms Locked drive on each computer. Also adheres toCARF and DMHSAS standards and criteria, federal and state confidentiality rules and regulations, and any special funding source requirementsincluding Medicaid and HCA regulations.

Supports and promotes a service environment for individualsin need of behavioral health and/or co-occurring services thatisrecovery focused, welcoming, and attentive to the needs of individuals who may have experienced trauma in their lives. Attends annual core competencies traininginrelationshiptoindividuals with co-occurring disorders or who have experienced traumain their lives.

Establishes andmaintainsprofessional working relationships withsupervisorand peers. Caneffectively work within a team or group setting cohesively.

Monitors eligibility status of clients for Social Security, Title XIX (Medicaid), QMB, Medicare and other benefits for which the client may qualify for andseekstoassisteligible clients in the Health Home Services/enrollment.

Maintains up-to-date address, phone number, contact persons, and financial informationonall clients.

Establishes rapport with clients, assessing client needs;organizes,plans,anddelivers face-to-face client services; and conductsfollow upon the outcome of services provided.

Responsible formaintainingstrictestconfidentiality guidelines for interactionsregardingconsumer and family information.

Provide clinical services for at least 80% of caseload..

Utilizes the philosophy of Collaborative/Therapeutic Documentation to meet clinical record keeping documentation standards and ensure documentation is completed with the Consumer to the extent possible and all documentation is completed on the same day of the date of service. Reports anddocumentsservicesin accordance withagency policy and procedures.

Responsible forseeking outsupervisoryassistanceforadditionaltraining and to enhance skill set as needed.

Interactswith other professionals to discuss therapy or treatment,new resourcesor techniques, and to share information.

May berequiredto work evening or weekend hours.

May berequiredto provide services in a community setting.

Mustbeable toprovide client transportation using personal vehicle or agency owned van or small bus.

Performs other related duties as assigned.

EDUCATION and/or EXPERIENCE

  • Must meet ODMHSAS and HOPE’s requirementsfor Behavioral Health Case Manager II certification.
  • Case management experience in behavioral health or social services preferred.

CERTIFICATES, LICENSES, REGISTRATIONS

Required to obtain and maintain DMHSAS CM Certification.

Required to complete, Therapeutic Options or approved safetytraining and annual updates.

May berequiredto obtain andmaintainFirst Aid and CPR Certifications.

May berequiredto complete andmaintainWellness Coach Certification

In order toprovide client transportation,mustpossesscurrent, valid Oklahomalicense, meetHOPE insurance requirements. HOPE will annually obtaincopyof Motor Vehicle Record from the Department of Public Safety.

LANGUAGE SKILLS

Ability to read andcomprehendbasic instructions, correspondence, and memos. Ability to write correspondence to courts, schools, etc. Ability to effectively present information in one-on-one and small group situations to families, clients, and other employees of the organization. Ability to complete thorough andaccurateclinical documentation and writing in a medical record chart and complete needed paperwork.

REASONING ABILITY

Ability to apply commonsenseunderstanding to carry out instructionsfurnishedin written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Ability toseek outsupervisoryassistanceforadditionaltraining when needed and make application of techniques or other skills to meet essential job duties and responsibilities.
