ViaQuest Psychiatric & Behavioral Solutions in Springfield, OH seeks a Behavioral Health Case Manager to support clients across ages with coordinated mental health services. The role includes therapy-related service planning, crisis prevention, and collaboration with a multidisciplinary team.

Responsibilities include providing services to individuals or groups, promoting constructive relationships with outside contacts, and traveling within the assigned service area.

Qualifications

• Bachelor's or Master's level degree in a related field or a high school diploma or three or more years of relevant field experience.
• Related experience or training with mental or behavioral health conditions.
• Valid driver's license, car insurance and reliable transportation.
• Willingness to travel throughout assigned service area.

Responsibilities

• Provide Therapeutic Behavioral Services after consulting with a licensed clinician.
• Plan individualized supports and develop treatment plans.
• Link clients with interventions and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.
