Job summary

Centers for Independence in Milwaukee seeks a dedicated Case Manager to support individuals living with severe mental illness and/or substance use disorders. You will build client relationships, craft personalized plans, and link clients to housing, healthcare, and community resources.

You’ll work with an interdisciplinary team to coordinate care, provide counseling, and monitor progress. Strong communication, reliability, and ability to navigate crisis situations are essential.

Qualifications

• Bachelor’s degree in behavioral science or related field.
• 1000 hours of post-degree clinical experience with individuals with severe and persistent mental illness.
• Valid Wisconsin driver’s license and vehicle access.
• Experience with Milwaukee County TCM/CCS/CSP-ACT Case Management preferred.
• Knowledge of motivational interviewing and community-based supports.

Responsibilities

• Build relationships and develop individualized, client-centered treatment plans.
• Provide supportive counseling, psychosocial rehabilitation, and crisis intervention.
• Coordinate care with an integrated treatment team.
• Connect clients with healthcare providers, housing, and community resources.
• Support medication management and monitor client needs.
• Advocate for clients and educate families and supports.
• Help clients develop skills for stability and independence.
• Complete assessments, treatment plans, case notes, and required documentation.
