Job summary

Old Colony YMCA in Hyannis, MA is seeking a Case Manager to oversee the delivery of quality casework at the OCC Community Justice Support Center. The role emphasizes honesty, respect, caring and responsibility in fulfilling duties.

You will conduct intake assessments, develop treatment plans, coordinate referrals and maintain participant records in CIMS. The position includes supervising interns/volunteers and facilitating CBT group sessions.

Qualifications

• Bachelor’s Degree from an accredited college or university.
• Two (2) years of supervised counseling experience or four (4) years of experience delivering manualized cognitive behavioral treatment.
• Must be willing to work with high risk populations including substance abusers, court involved individuals, and trauma survivors.
• Preferred licensed by professional board of practice in counseling, mental health, social work, or alcohol and drug abuse.
• Good oral, written, and presentation skills.
• Strong computer skills including Microsoft Word, Excel, Teams, Outlook and Power Point.
• A minimum of one evening shift per week is required.
• Some traveling required for meetings and trainings.
• Must have a valid driver’s license in good standing.
• Must submit and pass drug testing upon hire.
• Must be able to sit, stand, bend and walk during the work day.

Responsibilities

• Ensure casework services align with OCC policies and regulations.
• Complete intake assessments and develop treatment plans for each participant.
• Provide monthly reviews of treatment plans and goals.
• Make referrals to outside agencies and case-manage community-based services.
• Document participant interactions and track client progress.
• Create discharge plans and oversee post-care processes.
• Facilitate CBT groups and conduct group audits.
• Maintain participant data in the Client Information Management System (CIMS).
• Support supervision of interns, volunteers, and new staff.
• Attend meetings and trainings as requested by supervisors.
