Job description

Recovery Manager - Miami and surrounding counties.

CareStar, Inc. is seeking a Recovery Manager in the Operations department to conduct eligibility evaluations for individuals applying for specific programs and to create person-centered service plans. The Recovery Manager will coordinate all required program services and assist individuals in accessing Medicaid and other medical, social, educational, and community resources, regardless of funding source.

The Position

CareStar is looking for motivated individuals who:

  • Maintain knowledge of Medicaid, the behavioral health system, and community resources.
  • Conduct face-to-face eligibility evaluations, including administering the Adult Needs and Strengths Assessments, verifying residence in approved environments, and confirming behavioral health diagnoses, ensuring compliance with legal, program, contract, and CareStar requirements.
  • Develop, monitor, and update person-centered care plans to address individual needs, preferences, health, and welfare.
  • Coordinate and authorize specialized recovery services and program services.
  • Assist individuals in connecting with necessary resources, including Medicaid, medical, social, educational, and other services, regardless of funding source.
  • Adhere to the CareStar Rule and comply with all policies and procedures.
  • Maintain confidentiality of patient information, with unlawful disclosures treated as serious disciplinary offenses.
  • Follow the Acceptable Use Policy when using CareStar's information systems.
  • Possess a Bachelor's Degree in social work, counseling, psychology, or a related field, or be a Licensed Social Worker (LSW).
  • Have a minimum of one year post-degree experience working with individuals with diagnosed chronic conditions, or at least three years working with individuals with severe and persistent mental illness.
  • Possess knowledge of issues affecting people with severe mental illness and community-based interventions/resources.

This position is based in Miami County and covers surrounding counties.
