Job description

Recovery Manager – Lucas and surrounding counties.

CareStar, Inc. is currently seeking a Recovery Manager in Operations to conduct eligibility evaluations for individuals applying for specific programs and create a person-centered service plan. The Recovery Manager will coordinate all the program required services and assist individuals in gaining access to needed Medicaid services, as well as medical, social, educational, and other resources, regardless of funding source.

THE POSITION

CareStar is looking for motivated individuals who:

  • Maintain knowledge of Medicaid, the behavioral health system, and community resources.
  • Conduct face-to-face eligibility evaluations, including but not limited to the administration of the Adult Needs and Strengths Assessments; verification that the individual resides in a program approved environment; and verification of the individual's qualifying behavioral health diagnosis.
  • Develop, monitor, and update individual person-centered care plans to ensure the individual's needs, preferences, health, and welfare.
  • Coordinate and authorize all specialized recovery services program services.
  • Assist individuals with connections to needed resources, including Medicaid services, medical, social, educational, and other resources, regardless of funding source.
  • Adhere to the CareStar Rule in performance of job responsibilities.
  • Understand and comply with CareStar Policies and Procedures.
  • Maintain confidentiality related to patient information.
  • Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
  • Bachelor's Degree in social work, counseling, psychology, or a related field; or Licensed Social Worker (LSW).
  • Minimum of one-year post-degree experience working with individuals with diagnosed chronic conditions or minimum of three years’ post-degree experience working with individuals with severe and persistent mental illness.
  • Knowledge of issues affecting people with severe and persistent mental illness and community-based interventions/resources for those individuals.

This position is open in Lucas County and is responsible for covering the surrounding counties.
