Position Overview

Recovery Manager - Cuyahoga and surrounding counties.

CareStar, Inc. is seeking a Recovery Manager in Operations to conduct eligibility evaluations for individuals applying for specific programs and to create person-centered service plans. The Recovery Manager will coordinate all required services, assist individuals in accessing Medicaid and other medical, social, educational resources, regardless of funding source.

Responsibilities

• Maintain knowledge of Medicaid, behavioral health systems, and community resources.
• Conduct face-to-face eligibility evaluations, including assessments and verification of diagnoses and residence.
• Develop, monitor, and update person-centered care plans.
• Coordinate and authorize recovery services.
• Assist individuals in connecting with necessary resources.
• Adhere to CareStar policies, confidentiality, and acceptable use policies.

Qualifications

• Bachelor's Degree in social work, counseling, psychology, or related field; or Licensed Social Worker (LSW).
• Minimum one-year post-degree experience working with individuals with chronic conditions and/or three years with severe mental illness.
• Knowledge of issues affecting mental health and community interventions.
