Job summary

Centerstone is seeking a Case Manager to coordinate services and care for patients, offering coaching and strategies to improve health and wellness. You will work across clinic, home and community settings, applying trauma-informed practices to support whole-person care.

Ideal candidates hold a bachelor's degree in counseling, psychology, social work or related field, with strong communication skills and a passion for helping people coping with mental health issues.

Qualifications

• Education: Bachelor's degree in psychology, social work or related field.
• Experience with Electronic Health Records preferred.
• Valid driver's license in good standing required.

Responsibilities

• Help patients navigate the health care system through coaching, education and support.
• Coordinate behavioral and primary care needs and develop strategies to remove barriers to wellness.
• Refer and link clients and families to additional services as identified.
• Use communication and counseling techniques to develop daily living and coping skills.
• Develop, evaluate, and update treatment plans with input from clients and families.
• Document services in the Electronic Medical Record promptly.
• Meet clients in person, via telehealth or by phone to address needs.
• Attend staff meetings and participate in clinical supervision.

SUMMARY OF POSITION

Jump start your career in the mental health field as a Case Manager. You'll coordinate services and care for patients - providing coaching, strategies and coordination of health and wellness resources so they can live their lives more fully. Through your work, you will teach people the importance of the connection between physical and mental health.

ESSENTIAL DUTIES & RESPONSIBILITIES

• Help patients navigate the health care system through coaching, education and support
• Coordinate the behavioral and primary care needs and develop strategies to remove barriers to wellness
• Refers, advocates, and links client and family for additional services as identified.
• Use behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills
• Provides supportive and educational case management services to develop daily living and coping skills.
• Responsible for developing, evaluating, and updating treatment plans with input from the client and family.
• Enters written documentation into Electronic Medical Record in a timely manner for all services rendered.
• Connect with clients face to face in the office and community, telehealth, and by phone to address needs
• Demonstrate knowledge and understanding of ethical standards by evidence of practice with patients and community partners.
• Attend weekly staffing and team meetings and completes assessments
• Assist clients with developing natural support systems.
• Completes all required agency trainings to maintain 100% compliance.
• Maintains departmental productivity expectations.
• Attend and participate in clinical supervision.

QUALIFICATIONS

• Education Level: Bachelor's Degree in psychology, social work or related field required.
• Years of Experience: Experience with Electronic Health Record preferred.
• Certification/Licensure: Valid driver's license in good standing required.
