Job summary
Southern Highlands Community Mental Health Center in Mercer County, West Virginia is seeking a compassionate Case Manager/Care Coordinator Level 2 to help individuals access behavioral health, medical, social, educational, and community resources. This role coordinates services with multidisciplinary teams and supports recovery-oriented care.

Qualifications
- Bachelor's or Master's in a human services field per WV Board requirements.
- Licensed Psychologist (MS/PhD) considered.
- Licensed Registered Nurse also eligible.
- Experience in behavioral health or case management preferred.
- Strong communication, organizational and problem-solving skills.
Responsibilities
- Assess client needs and assist with service coordination.
- Develop and monitor individualized service plans.
- Connect individuals with community resources, housing, transportation, and education.
- Advocate for clients navigating complex systems of care.
- Maintain accurate and timely documentation per agency/state/federal requirements.
- Participate in treatment team meetings with internal and external providers.
- Monitor client progress and assist with goal attainment.
- Promote recovery-oriented, trauma-informed, and person-centered care.
