Job summary

LaSante Health Center in New York City is seeking a compassionate Case Manager to help patients address social, financial, and healthcare-related needs, coordinating with the clinical care team. You will connect patients with community resources and support services while assisting with applications, referrals, and documentation.

Responsibilities include assessment, care planning, follow-up, and accurate record-keeping to track progress and deadlines.

Qualifications

Experience in case management, social services, healthcare, or a related field preferred.
Familiarity with New York City community resources and public-benefit programs preferred.
Strong communication, organization, documentation, and follow-up skills.
Ability to work compassionately with vulnerable individuals and families.
High school diploma or equivalent required.
Bilingual skills are a plus.

Responsibilities

Assess patients' needs and develop individualized service plans.
Connect patients with food, housing, transportation, benefits, homecare, legal, and other community resources.
Assist patients with applications, referrals, and required documentation.
Follow up with patients and community organizations to ensure services are accessed.
Coordinate care with therapists, medical providers, and other team members.
Document all patient interactions, referrals, outreach, and follow-up activities accurately and promptly.
Track patient needs, referrals, deadlines, and progress.
