Job summary

Novant Health is seeking a BSW Case Manager to facilitate complex discharge planning and coordinate care with the healthcare team. You will collaborate with physicians and RN/MSW for guidance on complex cases and ensure safe, proactive discharge plans aligned with Novant Health policies.

You will act as a discharge planning and care coordination resource, maintaining knowledge of community resources and post-acute options to support patients and families throughout transitions of care.

Qualifications

• Bachelor's degree in Social Work from CSWE-accredited program.
• Two+ years professional social work or healthcare experience.
• NC state approved curricula for restrictive interventions within 3 months of hire.
• Weekend and holiday coverage required.
• Knowledge of Microsoft Office preferred.

Responsibilities

• Coordinate discharge planning and care coordination with the healthcare team.

What We Offer

The BSW Case Manager serves to facilitate complex discharge planning and is a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive, complex and safe discharge plans. Collaborates with RN/MSW Case Manager for guidance with medically and/or psychosocially complex cases. The BSW Case Manager along with the other members of the healthcare team accepts responsibility, authority and accountability for management of care. This shall be in accordance with the current policies and procedures, mission, vision and values of Novant Health. Serves as a resource for discharge planning and care coordination. Maintains current knowledge of available community resources and post acute care options.

What You'll Do

• Education: 4 Year / Bachelors Degree, required. Bachelor's Degree in Social Work from school accredited by the Council on Social Work Education.
• Experience: Two or more years professional social work experience or two or more years healthcare experience, preferred. Case Management or related experience, preferred.
• Licensure/Certification: NC state approved curricula for restrictive interventions (facility specific) within 3 months of hire for Emergency Departments and Behavioral Health units, required.
• Additional Skills/Requirements (required): Weekend and holiday coverage.
• Additional Skills/Requirements (preferred): Knowledge/Competence of Microsoft office products and keyboarding.
