Job summary

Novant Health is seeking a Case Manager MSW to lead patient assessment and complex discharge planning at New Hanover Medical Center in North Carolina. The role requires a Master’s in Social Work from a CSWE-accredited program, 2+ years of relevant experience, and licensure

Qualifications

• Graduate Degree in Social Work (MSW) from CSWE-accredited program.
• Two or more years of professional social work or healthcare experience; case management preferred.
• Licensure: state-approved curricula for restrictive interventions within 3 months of hire.

Responsibilities

• Lead patient assessment and complex discharge planning, coordinating with physicians and the healthcare team.
• Collaborate with RN Case Manager and other team members to develop proactive, safe discharge plans.
• Coordinate care and interventions addressing identified needs; ensure adherence to policies and quality metrics.
• Serve as a resource for discharge planning and post-acute care options, with knowledge of community resources.

What You'll Do

The Case Manager MSW serves as a leader of the patient assessment and complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive, complex and safe discharge plans and care plans. Collaborates with RN Case Manager’s for guidance with medically complex cases as needed. The Case Manager MSW is also responsible for facilitating coordination of care facilitating interventions to address identified need(s). The Case Manager MSW along with the other members of the healthcare team accepts responsibility, authority and accountability for management of care and quality metrics. This shall be in accordance with the current policies and procedures, mission, vision and values of Novant Health. Serves as a resource for discharge planning and care coordination for psychosocially complex cases. Maintains current knowledge of available community resources and post acute care options.

What We're Looking For

• Education: Graduate Degree, required. Master's Degree in Social Work from school accredited by the Council on Social Work Education.
• Experience: Two or more years professional social work experience or two or more years healthcare experience, preferred. Case Management or related experience, preferred.
• Licensure/Certification: Appropriate state approved curricula for restrictive interventions (facility specific) within 3 months of hire for Emergency Departments and Behavioral Health units, required.
• Additional Skills (required): Weekend and holiday coverage.
• Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding.
