Job Summery:
The case manager is responsible for delivering case management services to eligible prenatal, postpartum, and parenting families under the Healthy Start program. Healthy Start serves pregnant women and families with children up to 18 months old who live in Dauphin County. These services aim to improve access to medical care and community resources, promote healthy behaviors before, during, and after pregnancy, and encourage the use of safe and healthy infant and toddler care practices.
Essential Functions:
- Conducts visits every 2 weeks or monthly with assigned participants at their home, in the community, or at the office.
- Perform case management activities, including screening and assessments using the Healthy Start data collection tools, developing and monitoring and individualized service care plan, connecting clients to community resources, facilitating and coordinating timely referrals, tracking health information, conducting risk evaluations, advocating for clients, and providing emotional support.
- Provide evidence-based education to all participants on topics related to pregnancy, the postpartum period, child development, risk reduction, and healthy behaviors.
- Maintain professional boundaries and ethical conduct in all interactions.
- Attend all necessary department and team meetings.
- Ensure compliance with paperwork and contact requirements for all assigned clients as required by program funding organizations and other regulatory agencies.
- Enter all required documentation electronically into the appropriate database accurately and efficiently.
- Complete and keep track of continuing education credits as required by program regulations.
Requirements:
- Case Management Knowledge and Skills: A fundamental understanding of case management functions and elements.
- Tech-Savvy: Proficiency in basic data entry computer skills and the ability to learn new software of databases specific to the role.
- Strong Interpersonal Skills: Ability to communicate courteously and effectively with individuals from diverse backgrounds.
- Team Player: An industrious and cooperative nature to foster harmonious working relationships with colleagues, leadership, community partners and community members.
- Education: Bachelor’s degree in social work, psychology, sociology, or another related field is preferred. In place of a related degree, candidates with a bachelor's degree in a non-similar field or any combination of relevant experience and education related to the functions of the job will be considered.
- Bi-lingual: Fluent in both English and Spanish
- Valid Driver's License: Ability to travel throughout the service area
- Prior CPR/First Aid certification
Preferred Qualifications:
- Two years of working experience in the case management or comparable social services role.
- Prior experience working with pregnant, postpartum, or parenting families in a community based setting
- Knowledge of The Joint Commission regulations
- Knowledge of community resources in Dauphin County and surrounding areas
