Responsibilities include completing functional assessments, developing ICMPs, coordinating with clinical staff, attending meetings, ensuring licensure compliance, and supporting families.

Requirements: Bachelor’s degree in social work, psychology, or related field, at least one year of DD/ID

Qualifikationen

• Bachelor's degree in social work, psychology, counseling, or related field.
• At least one year of DD/ID experience or specialized training preferred.
• Minimum age of 21 per licensure requirements.
• Valid Indiana driver's license and driving record that meets eligibility requirements of Damar's insurance carrier.
• Be clear of any felony convictions, substantiated charges of neglect or other disqualifying criminal convictions.
• Pre-employment drug screen and physical.
• Background screening requirements per Damar policies.

Aufgaben

• Conducts functional assessment and develops ICMPs for each client at intake and updated as needed.
• Leads and directs implementation of ICMPs, providing guidance and training to staff.
• Participates in psychiatric reviews with input to psychiatrist during appointments or via nursing.
• Completes CANS assessments for residents per state regulations.
• Oversees Family Time scheduling and encourages family involvement.
• Organizes program meetings and Family-Care Team meetings when necessary.
• Attends mandatory staff meetings and program meetings.
• Ensures compliance with agency policies and procedures.
• Participates in data collection to improve outcomes and service delivery.
• Completes intakes and discharges for assigned clients.
• Conducts and documents Mental Health Assessments.
• Coordinates Independent Living and education coordination.
• Updates client information in systems.
• Represents clients at appointments, court reviews, and IEP/Case Conferences with progress notes.
