Job summary

TASC is seeking a compassionate and motivated Case Manager to support clients involved in the criminal legal system who struggle with substance use and behavioral health challenges. Case Managers in our Adult Criminal Justice and Treatment Services division provide assessments, referrals, and case management to support recovery and long-term stability.

Essential duties include assessments, intake, referrals, and collaboration with partners to ensure coordinated care.

Qualifications

• High school diploma or GED required.
• Proficient with DSM-5 and ASAM criteria as it relates to substance-related disorders.
• Comfort working inside a correctional setting and with individuals convicted of serious offenses.
• Must be able to pass an IDOC Background check.
• Experience with group facilitation and coordinating referrals.

Responsibilities

• Conduct comprehensive assessments and develop service plans tailored to client needs.
• Facilitate intake and enrollment into TASC programs.
• Provide referrals and crisis intervention for behavioral health, housing, employment, and other supports.
• Collaborate with community partners and internal staff to ensure coordinated care.
• Apply DSM-5 and ASAM criteria in evaluating client needs and treatment referrals.
• Maintain timely and accurate documentation, including progress notes, reports, and discharge summaries.
• Monitor progress toward service goals and ensure compliance with all reporting systems.
• Participate in staff meetings, trainings, and multi-disciplinary team discussions.
• Support clients in navigating systems and accessing essential services.
