Beth Israel Lahey Health is seeking a Part-time Case Manager at Danvers Treatment Center to support detox patients with intake, education, and coordination of care. This role offers 32 hours per week on the first shift and benefits starting on day one.

The ideal candidate has experience in addiction treatment and strong counseling skills. Responsibilities include intake evaluations, group activity support, client evaluations, and arranging aftercare referrals and transportation as needed.

Formación

• Associates or Bachelor’s Degree in Human Services or related field preferred; High School Diploma or Equivalent required.
• A minimum of one year related experience and/or training in addiction treatment.
• Valid Driver’s License in good standing preferred.

Responsabilidades

• Obtains documents and intake materials; conducts intake evaluation and counseling sessions.
• Shares responsibility for education and other group programming activities.
• Participates in client evaluations and case conferences.
• Provides referrals for affected family members to self-help programs when applicable.
• Assesses continuing care needs and arranges transfer to appropriate rehab facilities or liaises with self-help organizations.
• Coordinates transportation to next level of care.

Danvers Treatment Center is a 41 bed detoxification program dedicated to helping men and women by providing 24 hour nursing care to monitor an individual’s withdrawal from alcohol and/or other drugs and alleviate symptoms. Clients are provided with the medical and emotional support that they need during the one of the most pivotal stages in the recovery process. Services include individual case management and group treatment services focused on assisting the client with accessing step-down levels of care. The Case Manager role is a great opportunity for an individual who wants to advocate for and serve individuals who struggle with addiction within a collaborative, established, patient-centered organization.

Job Description

• Scheduled Hours: 32 Hours, 1st Shift

The Case Manager Responsibilities Are

• Obtains documents and all relevant intake material. Conducts individual intake evaluation and counseling sessions,
• Shares responsibility for education and other group programming activities.
• Participates in client evaluations and case conferences.
• Provides referrals for affected family members to self-help programs when applicable.
• Assesses patients’ continuing care needs, contacts appropriate aftercare resources and arranges for transfer or admission to appropriate rehabilitation facilities and/or liaison with self-help organizations.
• Coordinate client’s transportation to next level of care.

Qualifications

• Associates or Bachelor’s Degree in Human Services or related field preferred; High School Diploma or Equivalent required.
• A minimum of one year related experience and/or training in the area of addiction treatment.
• Valid Driver’s License in good standing preferred.

Pay Range

$21.77 - $26.54

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
