Qualifications

Responsibilities

• Works with physicians and multidisciplinary team to develop a plan of care for assigned patients.
• Monitors patient progress and ensures progress towards outcomes; identifies barriers to care.
• Actively involved in discharge planning and integrates hospital mission into daily practice.
