Youth Opportunity & Lake Parrish Youth Academy in Wimauma, FL seeks a Case Manager to provide comprehensive behavioral health case management for youth aged 14–18 in residential programs. Primary duties include coordinating assessments, treatment plans, and family communication, with a focus on compliance and outcomes.

The role requires a Bachelor’s degree in a related field, a valid driver’s license, and the ability to pass background checks.

Responsabilidades

• Provide case management services, including communication with youth, families, and Juvenile Probation Officers.
• Complete or coordinate required mental health and substance abuse assessments.
• Develop and update treatment plans, aftercare plans, performance summaries, and discharge summaries.
• Support data collection for post-release tracking and outcomes.
• Serve as liaison between the program, DJJ, families, and external agencies.
• Prepare families for visitation, home leave, aftercare, and group/family meetings.
• Connect youth and families to appropriate community resources.
• Participate in internal audits and reviews to ensure policy compliance.

Job Type: Full-Time | Monday–Friday (Some Saturdays and Sundays)
Pay: $45,000 - $50,000 based on experience
Schedule: Primarily weekdays with flexibility for program needs

About Youth Opportunity & Lake Parrish Youth Academy

Lake Parrish Youth Academy is comprised of multiple co-located residential commitment programs serving moderate risk maleyouth aged 14 to 18who require behavioral/mental healthand substance abuseservices. The programs utilize evidence-based practices to provide comprehensive care, treatment, and supervision. Youth will also receive psychotherapeutic services, including individual, family, and group counseling while at the program.

A youth’s length of stay is determined by the youth's assessed risk to reoffend, their progress in reducing applicable criminogenic needs and risk factors, and the completion of their individualized treatment plan.
