Job summary

Pinnacle Treatment Centers in Meadville, PA is seeking a dedicated Case Manager to support patients through their recovery journey. You will work with the Clinical Director and Executive Director to create and maintain a therapeutic environment and coordinate continuum of care for residents.

Responsibilities include coordinating assessments, treatment planning, crisis intervention, connecting patients with community resources, and assisting with daily living activities.

Qualifications

• Bachelor’s degree in Human Service field from an accredited college or university.
• Valid/Current Driver's License.
• Ability to travel up to 15% may be required.
• Preferred: Previous experience in case management and/or utilization review.

Responsibilities

• Provide case management and care coordination services for patients enrolled in treatment.
• Coordinate assessments, treatment planning, and crisis intervention when needed.
• Link patients to community resources and monitor service delivery and compliance with external sources.
• Assist patients with daily living activities and connecting to services (financial, housing, food, medical, education, vocational, etc.).
• Coordinate with family members, friends, and organizations; manage criminal justice service coordination where applicable.
• Conduct education sessions for patients and families; participate in discharge planning when needed.
• Attend team meetings and training; maintain confidentiality per policies.
• Other duties as assigned; ensure timely cross-team communication and adherence to policy.

As a Case Manager, you play a direct role in saving the lives of our patients each day by providing and promoting an environment of long-term recovery. Requirements:

Responsibilities:

• Providing case management and care coordination services for patients enrolled in treatment. Responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements.
• May include activities such as coordinating patients assessments, treatment planning and crisis intervention services; providing training and facilitating linkages for the use of community resources; monitoring service delivery; obtaining or assisting patients in obtaining necessary services (e.g. financial assistance, housing assistance, food, clothing, medical services, educational services, vocational services, recreational services, etc.); assisting patients in networks with family members, friends, and/or organizations; assisting patients in performing daily living activities; and coordinating criminal justice services.
• Conducting individual and family educational sessions and engaging in various duties related to coordinating treatment and discharge activities, as needed.
• Coordinating, in a timely manner, issues or activities relevant to the team, and/or to the team's relationship within the facility or treatment milieu
• Adhering to organizational policies, procedures, performance improvement initiatives and maintains organizational and industry policies regarding confidentiality.
• Attend team meetings and complete all training courses timely as required.
• Other duties as assigned.
