Qualifications

Responsibilities

  • Conduct intake sessions with new residents in the program.
  • Develop and maintain individualized case plans and care plans.
  • Provide regular case management and referrals for housing and services.
  • Coordinate with referring agencies and assist with transportation to appointments.
  • Deliver crisis intervention, risk assessment, and safety planning.
  • Supervise interns and oversee the internship program.

Job description

The Human Trafficking Case Manager is responsible for providing client-centered crisis intervention, client advocacy, safety planning, supportive counseling, case management, and referrals for adult survivors of trafficking. The HT Case Manager must be able to deliver agency services and initiatives in a culturally sensitive manner specific to the person’s race, religion, color, sex, age, national origin, disability, ethnicity, sexual orientation, gender, gender identity or expression.

The HT Case Manager works as a member of the Human Trafficking team to provide services to shelter, transitional housing, and community-based clients. The HT Case Manager reports directly to the Director of Human Trafficking Services.

Key Responsibilities:
Client Services
  • Conducts intake sessions with new residents in the program.
  • Works with clients to develop, implement, and maintain individualized case plans and working care plans for residents.
  • Provides regular case management and access to in-agency and external resources for shelter, transitional housing, and adult community clients.
  • Works closely with referring agencies, including but not limited to victim witness and the criminal justice system, on behalf of clients.
  • Transports and accompanies clients to appointments and court as needed; ensures agency van maintenance and upkeep.
  • Refers clients to other agencies and resources and advocates directly with other agencies as needed to help clients access necessary services.
  • Provides client-centered crisis intervention, risk assessment, supportive counseling, and safety planning.
  • Educates clients about the dynamics of human trafficking, sexual assault, and substance abuse.
  • Facilitates regular house meetings at shelter and transitional housing locations.
  • Facilitates psychoeducational groups, including but not limited to: WCR (Wellness, Connection, & Resiliency) Group, Seeking Safety Group, Community Group
  • Maintains strong communication with counseling and shelter team staff regarding client cases.
  • Performs routine drug and alcohol screening for clients.
  • Serves as part of the on-call team for the HT Shelter, along with other HT Leadership and the Director of Human Trafficking Services, to assist in emergency situations.
Program & Facility Responsibilities
  • Oversees maintenance coordination and general upkeep of HT emergency shelter and HT transitional houses to ensure safe and supportive living environments.
  • Ensures agency vehicle (van) maintenance, including scheduling services and maintaining cleanliness and readiness for client transportation.
  • Supervises interns and oversees the Human Trafficking intern program, including training, mentorship, and performance support.
Administrative Duties
  • Completes documentation of case management sessions and group facilitation.
  • Maintains strict confidentiality of client information in accordance with agency policies, funder requirements, and applicable federal and state laws.
  • Maintains client outcome database and prepares reports as needed.
  • Represents Safe Harbor at designated meetings and community collaborations.

  • Participates in weekly team meetings and monthly staff meetings.
  • Performs other duties as assigned.

  • Attends all required agency meetings and events.
  • Participates in regular training to enhance knowledge and skills, per accreditation criteria.
  • Assists other staff members on projects as needed.
Required Qualifications:
  • Bachelor’s degree in related field and/or five years of advocacy experience, preferred.
  • Strong organizational skills and experience managing multiple projects.
  • Experience using Microsoft Office software platforms.
  • Ability to communicate both orally and in writing with team members and clients.
  • Demonstrated understanding of sexual abuse and substance abuse.
  • Experience facilitating psychoeducational or therapeutic groups preferred.
  • Experience supervising interns or volunteers preferred.
  • Must work well with persons of varied sexual preference, racial, ethnic, cultural, socioeconomic backgrounds, and with individuals who are differently-abled.
  • Previous employment or volunteer work in human services, community agencies, or other related nonprofit or social change organizations a plus.
  • Ability to remain patient, calm, and professional when working in crisis situations, and able to make decisions independently and as a member of a team.
  • Required to work occasional evenings, weekends, and holiday hours.
  • Must have reliable transportation and excellent driving record.

$47,000 – $52,000 / year
