Laukoa seeks a Case Manager to support the State Surgeons Office for the New York Army National Guard. This role handles medical/dental records, assists service members with records, and provides admin support for readiness programs and events.

On-site in Watervliet, NY with travel across New York; full-time contract, M-F with some weekends. Role requires CAC/NACI background checks, HIPAA adherence, and proficiency with MS Word/Excel/PowerPoint/Outlook.

• Acquiring CAC and passing NACI background check required
• Must have reliable transportation
• Contract employees will be required to travel to locations throughout New York State to meet with service members several times a year
• Knowledge of Microsoft programs such as Excel Adobe PDF and Word is a must

Mansioni

• Track medical status of all assigned Soldiers
• Coordinate Soldier treatment plans with medical facilities
• Submit case processing medical documentation to providers
• Travel to MREs events as determined by state needs
• Maintain Soldier medical records per Army Regulation AR 40-66
• Educate Soldiers on resources to improve medical and dental readiness
• Update MODS for immunizations and pregnancy records
• Prepare cases for monthly review with Lead Case Manager

SUMMARY

Laukoa is seeking a Case Manager to support to the State Surgeons Office for the New Yord Army National Guard ARNG This role helps maintain accurate and accessible medical and dental records assists service members with obtaining and managing their records and provides administrative support for medical readiness programs and events The Case Management Assistant will work closely with Soldiers medical personnel and other offices to organize and process medical documentation research information respond to inquiries and support Soldier Readiness Processing SRP and Medical Readiness Weekend MRW activities This position is well suited for an organized detail oriented professional who enjoys helping others and working in a mission focused environment This is a full time contract position with a MondayFriday schedule and occasional weekend and in state travel throughout New York On site Watervliet NY 12189

Hourly Rate 3383 an hour plus 542 H&W applied to the first 40 hours worked PTO each week

REQUIRED

Acquiring CAC and passing NACI background check

DUTIES AND RESPONSIBILITIES

Responsible for tracking medical status of ALL assigned SoldiersCoordinate Soldier treatment plans with appropriate medical facilitiesSubmit case processing medical documentation and profile concerns to militarycontacted providerTravel to and attend MREs events as determined by the needs of the state and the individual unitsManage Soldiers care from the identification of the condition to when the Soldier is eitherConsidered medically readyAdministratively separated from the militaryEnsure all medical documentation received from the Soldier andor their medical team is received and placed inappropriated electronic recordMaintain Soldier medical records in accordance with Army Regulation AR 40 66 Medical Record Administration and Healthcare DocumentationEducate Soldiers on resources available to improve medical and dental readinessMaintain Health Insurance Portability and Accountability Act of 1996 HIPAA standards and guidelinesReview and track health assessment lab results and contact SM and educate on lifestyle modificationsUpdate MODS as required on the following immunizations designation of pregnancy eye exams and medical warning tagsTrack medical appointments by communication with unit to ensure Soldier attendance and submission of follow up documentationReport Medical Readiness to the Commander or hisher designated representativePrepare cases for and participate in monthly review of all open case files with Lead Case Manager

• MODS modules
• Medical Non Deployable MND Health Readiness Record HRR
• DENCLASS Medical Data Protection System
• MEDPROS Line of Duty LOD and DOJPAAS 9

QUALIFICATIONS

Desirable Skills

Experience in military especially Army National Guard or veterans populations
