Job summary

Hcshiring is seeking a field-based social services professional to coordinate community-based services for members and keep accurate records. You will plan and contact members, liaise with informal caregivers, and ensure services align with care plans.

The role involves conducting in-home visits and assessments, documenting findings, and resolving variances. You will meet with members' Primary Care Physicians quarterly to discuss progress and adjust plans as needed, fostering collaborative care.

Qualifications

• BS in social work or related human service field is required.
• Experience in social work, home and community based services, healthcare or geriatrics preferred.
• Valid Driver's License.
• Reliable transportation.
