Job summary

Indiana Professional Management Group is hiring a full-time Case Manager to serve Huntington County. This hybrid role combines remote work from your home with local travel for community meetings several times a week.

You will help individuals navigate Medicaid waivers, develop support plans, coordinate services, and attend team meetings with providers. A Bachelor's in a human services field and relevant experience are required; a valid driver's license and reliable transportation are essential.

Qualifications

• Bachelor's degree in a human services field (psychology, sociology, social work, etc).
• At least 1 year of direct experience with individuals with intellectual/developmental disabilities or 2 years with aging population.
• Valid driver’s license, car insurance, and reliable transportation.
• Ability to pass background screening.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual Support Plan based on strengths, needs, goals, and desires of the individuals served.
• Help individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Let’s talk about the Case Manager position!

This full-time, hybrid position combines remote work from your home office with travel of up to approximately one hour for community-based meetings several times a week. This position serves Huntington County, and to accommodate required in-person meetings, applicants must reside within Huntington County, Indiana. Candidates must be able to travel throughout the county and surrounding counties as needed to attend community-based meetings.

What You’ll Do:

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Who You’ll Work With:

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

• A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field,
• and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.

• A valid driver’s license, car insurance, and reliable transportation
• The ability to pass thorough background screens
