Job summary

Indiana Professional Management Group, Inc is seeking a Case Manager to provide education on Medicaid waivers and design individualized support plans. The role blends remote work from a home office with in-county travel for meetings in Harrison County, Indiana.

Applicants must reside in Harrison County and be able to travel across the county and nearby areas for in-person meetings. A Bachelor’s degree in human services or related fields is preferred, with related experience required for

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or
• A Bachelor’s degree in an unrelated field and a minimum of one year of direct experience with individuals with intellectual/developmental disabilities, or two years experience with aging population.

Responsibilities

• Create an Individual Support Plan based on strengths, needs, goals, and desires.

Case Manager Position

This full-time, hybrid position combines remote work from your home office with travel of up to approximately one hour for community-based meetings several times a week. This position serves Harrison County, and to accommodate required in-person meetings, applicants must reside within Harrison County, Indiana. Candidates must be able to travel throughout the county and surrounding counties as needed to attend community-based meetings.

What You'll Do

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Who You'll Work With

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

Requirements

• A Bachelor\'s degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or
• A Bachelor\'s degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population
• A valid driver\'s license, car insurance, and reliable transportation
• The ability to pass thorough background screens
• Designated home office setting with computer, internet service, and smart phone
