Job summary

Ernest Health, Inc. is seeking a Case Manager for our Thornton, Colorado location to join a dedicated team providing inpatient rehabilitative services.

The role supports comprehensive patient care plans, discharge coordination, and collaboration with physicians and therapists in a busy hospital setting. • One year recent relevant experience required; acute care setting preferred.

Responsibilities

• Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients.
• Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records.
• Identifies and resolves barriers that hinder effective patient care.
• Actively involved in discharge planning process.
• Integrates the hospital’s mission and “Guiding Principles” into daily practice.

Qualifications

Required Skills:

• Current clinical licensure strongly preferred.
• One year recent relevant experience required. Acute care setting preferred.
