Job summary

SSM Health Rehabilitation Hospital in Bridgeton, MO is seeking a Case Manager. This full-time role coordinates care decisions, discharge planning, and cross-continuum collaboration to improve outcomes while ensuring cost-effective hospital-based services.

You will work a Monday–Friday schedule with 8-hour days and a compensation range of $77,000 to $105,000, depending on experience. Benefits include generous PTO, health and retirement plans, and a strong orientation program.

Qualifications

• Current licensure per state guidelines in clinical or related discipline (RN preferred) or a Bachelor's or Master's in a health or human services discipline.
• Previous experience in Case Management and Discharge Planning preferred.
• CCM certification preferred.

• Coordinate health care decisions and discharge planning to improve quality and outcomes.
• Manage care planning, fiscal oversight, and payer/referral coordination.
• Promote cost-effective care and ensure appropriate hospital-based services and utilization.

Responsibilities

The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of hospital-based services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans. The Case Manager assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.
