Role Description

This full-time Case Manager role is a field position based in the Greater South Seattle Area and Pierce County, with a blend of in-person work and some office work flexibility. The Case Manager will assess clients’ needs, develop individualized care plans, and coordinate services across healthcare providers, caregivers, and senior living communities. Daily activities include conducting client assessments, facilitating care transitions, monitoring care quality, and advocating for clients and their families, and tours. The role also involves maintaining accurate documentation, collaborating with interdisciplinary teams, and providing education and resources to families navigating caregiving and placement decisions.

Qualifications

• Experience in care management, case coordination, or social services, preferably with older adults and their families.
• Knowledge of senior living options, community resources, and caregiving support services in the Greater Seattle Area.
• Strong communication, counseling, and conflict-resolution skills to support clients and families during complex care decisions.
• Ability to manage multiple cases, prioritize tasks, and maintain organized and thorough documentation.
• Comfort with work, including in-home visits, facility visits, and communication and documentation. In office meetings and work as well.
• Basic proficiency with computers, electronic records, and online research tools.
• Relevant licensure or certification (e.g., RN, LPN, social work, or care management certification) preferred.
• Prior experience in nursing, gerontology, social work, or a related healthcare field is highly beneficial.
