Job summary

Family First at Home, a licensed home care agency in Washington, is seeking an experienced Registered Nurse to provide in-home client care and coordinate services with families.

This field-based role offers flexible schedule (Full-time or Part-time; 8:00am–4:30pm) and requires travel across Pierce and King Counties; you will conduct assessments, develop care plans, and advocate for clients.

Qualifications

  • Current, unrestricted Washington RN license.
  • At least 3 years of direct patient care or clinical experience.
  • Strong assessment, documentation, communication, follow-through, and time-management skills.
  • Ability to work independently in clients’ homes and throughout the community.
  • Valid driver’s license, current auto insurance, and reliable transportation.

Responsibilities

  • Assess clients in their homes and complete initial and ongoing assessments of their health, safety, support systems, and daily needs.
  • Create Personalized Care Plans with clear, measurable goals and update those plans as the client’s needs change.
  • Coordinate care across clients’ homes, medical offices, hospitals, rehabilitation centers, senior living communities, and community providers.
  • Help clients and families understand their options and connect them with home health, hospice, home care, financial assistance, housing, legal aid, and other community resources.
  • Support safe transitions from hospitals, rehabilitation centers, or senior living communities back home or to another care setting.
  • Advocate for clients during appointments, care conferences, crises, and changes in condition.
  • Provide nursing education and guidance within the scope of the role, including helping families recognize changes that may require clinical follow-up.
  • Coordinate with physicians, pharmacies, home health and hospice teams, and other clinical providers.
  • Stay in regular contact with clients and families and work closely with caregivers, providers, and the internal care team.
  • Document assessments, care plans, client contacts, progress, billable time, and other required information promptly and accurately.
  • Meet the position’s billable-hour and documentation expectations.
  • Transport clients to medical appointments or community resources when needed.
  • Participate in the Care Manager on-call rotation as assigned.

Location: University Place, WA - Hybrid — home, office, and local travel throughout Pierce and King Counties
Job Schedule: Full-time or Part-time; Monday through Friday, within the hours of 8:00am – 4:30pm, plus participation in the on-call rotation as assigned
Pay: $50.00 – $58.00 per hour, based on experience

About The Role

Family First at Home is looking for an experienced Registered Nurse who enjoys working with clients and families in their homes and communities. This role is a good fit for an RN who likes solving practical problems, coordinating services, and remaining involved as a client’s needs change.

Our Professional Care Managers support older adults and other clients living with chronic, acute, or terminal illnesses. You will manage a caseload, complete assessments, develop care plans, coordinate services, and help families work through healthcare and caregiving decisions that can feel overwhelming.

This is an independent, field-based role, but you will have a team available for consultation and collaboration.

Job Duties

Preferred Experience
  • 2+ years of experience in home health, hospice, home care, public health, community-based care management, or a similar setting.
  • Experience supporting older adults, people living with chronic or terminal illnesses, and family caregivers.
  • Experience with discharge planning, care transitions, advance care planning, or crisis response.
  • Certified Care Manager credential, such as CCM or CMC.
Travel and Mileage Reimbursement

Regular travel throughout Pierce and King Counties is part of this position. Business mileage incurred while traveling between work locations or visiting clients and referral partners will be reimbursed in accordance with the company’s mileage reimbursement policy.
