Job summary

APA Family Support Services in San Francisco, CA is seeking a dedicated case manager/family advocate to support families referred by CPS and partner agencies, and to participate in community events. You will carry a caseload, coordinate intake, assessment, and referrals, and liaise with providers to ensure resources reach families.

The role requires a BA in social services or related field, strong interviewing and crisis intervention skills, and proficiency with client records systems.

Qualifications

  • Bachelor’s degree in social services, psychology, or related field (or equivalent coursework/experience).
  • Experience in interviewing, crisis intervention, and case management.
  • Knowledge of San Francisco child welfare system and social services.
  • Excellent interpersonal and communication skills.
  • Ability to work in a team and lead when needed.
  • Computer skills including word processing and spreadsheets.
  • Bilingual in Cantonese, Lao/Thai, or Spanish is a plus.

Responsibilities

  • Provide crisis intervention and case management to families; carry a caseload of 12-16 families.
  • Coordinate intake, assessment, and referrals to internal and community providers.
  • Facilitate children and families events and groups; liaison with service providers.
  • Maintain client records in Salesforce and CMS; perform progress assessments.
  • Prepare project reports; maintain documentation for funding and compliance.
  • Engage in outreach to promote program services and recruit participants.
  • Develop outreach materials and agency communication materials.
  • Represent APA in agency and community meetings.

Job description

Salary: $53,000-$56,000 annually with generous vacation and holiday leave, Health, dental, and vision insurance,

Duties and Responsibilities:

This position will be responsible for providing case management/family advocacy through self-referral and from other service providers, referred by CPS (Child Protective Service) and other agencies, Child and Family Team meetings, and supervised visitation services; and participate in community events and activities.

  • Provide crisis intervention and case management to families. Carry a case load of 12-16 families.
  • Coordinate services to include intake, assessment, and referrals to internal services and community service providers.
  • Facilitate children and families events and groups.
  • Liaise with service providers to coordinate resources and services for families and to follow up with linkage to services offered.
  • Maintain client records in Salesforce and CMS and perform assessment evaluations to update client progress.
  • Develop agency material including project flyers and other outreach materials.
  • Participate in agency meetings and community meetings as directed. Conduct outreach and recruitment activities to the targeted population to effectively promote program services.
  • Maintain required documentation for funding and statistical compliance.
  • Prepare project reports and other documents promptly.
  • Maintain ongoing relationships with community service providers, funders, and other agencies/programs to educate and advocate for the needs of families and communities.
  • Perform other duties as may be assigned.
Requirements:
  • Bachelor’s degree in social services, psychology, or a related field; and or a combination of coursework and field experience in areas of children and family services, and social work with child abuse prevention focus.
  • Interest and commitment in the field of family support or community services.
  • Demonstrated knowledge and skills in interviewing techniques, crisis intervention, and case management services.
  • Strong organizational, decision-making, and problem-solving skills.
  • Knowledge of & familiarity with San Francisco’s child welfare system & social services community.
  • Excellent interpersonal and communication skills.
  • Ability to work as a team member and leader.
  • Computer technical skills including word processing and developing spreadsheets.
  • Bilingual in Cantonese, Lao/Thai, or Spanish is a plus.
  • Ability to lift and/or move up to 25 pounds withphysicalcapability to perform job-related duties

APA Family Support Services (APA) is a community-based organization that focuses on the prevention and identification of child abuse/neglect in the community. Our mission is to promote healthy children and families by providing family support services to prevent child abuse and domestic violence and to advocate for culturally competent services through education, community building, and leadership development.

We do this through case management and family support services at our family resource centers. By providing parents with a safe and welcoming place outside of their homes, they increase their social circles and have a forum to gain their voice.
