Pyramid Healthcare’s New River Valley Inpatient program in Radford, VA is hiring a Full-Time Case Manager. The role centers on coordinating care, connecting clients to resources, and planning for aftercare to support lasting recovery.

Salary is $18–$22 per hour, commensurate with education and experience. Join a team focused on continuity of care, collaboration with clinical staff, and helping families prepare for life after treatment.

Qualifications

• Bachelor's degree in a human services-related field with at least 2 years of post-degree direct care experience serving the population.
• Bachelor's degree in a non-human services-related field with at least 4 years of post-degree direct care experience serving the population.
• Strong communication, organization, and relationship-building skills.
• Commitment to client confidentiality, teamwork, and supporting individuals throughout their recovery journey.

Responsibilities

• Coordinate client care, appointments, treatment schedules, and aftercare services.

As a Case Manager at Pyramid Healthcare, you'll help individuals navigate treatment, connect with essential resources, and build a plan for success beyond our program. From coordinating services and supporting families to developing aftercare plans, your work will help clients take meaningful steps toward lasting recovery.

What You'll Do
