Job summary

Southwest Behavioral Health Services is seeking a dedicated Case Manager to coordinate care, conduct assessments, and connect adults with community resources in Prescott Valley. You will work with an interdisciplinary team to ensure coordinated, person-centered services that improve lives and communities.

Qualifications

  • Minimum age of 22.
  • Bachelor’s degree in behavioral health with one year of relevant experience.
  • Combination of experience and education totaling 2 years is acceptable.
  • Valid AZ driver’s license & motor vehicle clearance.
  • Fingerprint clearance issued by AZ DPS or eligible to receive on hire.
  • 3 weeks of PTO in first year with accruals.

Job Preview at a Glance:

The Case Manager serves as a central support within the interdisciplinary team, working primarily with adults. This staff member conducts assessments, assists members with resources within the community and provides coordination of care inside and outside of the agency.

Pay Rate: $17.50-$18.00 hourly

Responsibilities
  • Intake screenings (Completing intake paperwork and referrals)

  • Valid AZ driver’s license & motor vehicle clearance.
  • Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive upon hire.
  • 3 weeks of PTO your 1st year of employment, with increased accruals after continued service!

Education

Bachelors or better in Social Work or related field.
