Job summary

Hand of Mercy Health Care seeks a Case Manager to support clients with mental health needs in Portland, Augusta and Lewiston. The position is 28–34 hours per week, paying $33.00 per hour, with paid training, meetings, supervision, travel time and documentation.

Eligible for benefits including health and vision insurance, 401(k) and paid time off. The CM will assist clients with coping skills, coordinating services, building supports, and ensuring follow-through on medical and mental health

Qualifications

• 18 years or older with bachelor’s degree.
• MHRT/C certification required.
• Reliable transportation with insurance coverage.
• Ability to maintain at least 28 billable hours weekly.
• Eligibility to work in the USA.

Responsibilities

• Assess, plan, implement, monitor, and evaluate client health and needs.
• Create and implement treatment plans and coordinate services.
• Support clients in building resiliency and accessing community resources.
• Maintain accurate progress notes and documentation in the EHR.
• Travel locally to meet clients and attend trainings as needed.

Case Manager

HOM has a case management position available in Portland, Augusta and Lewiston 28-34 hour/week. $33.00 hourly, plus paid training, meetings, supervision, travel time, and documentation. Employee will receive additional 2 dollars increase within 2 years.

Job Description:

The CM will assist and support clients who are diagnosed with a mental health diagnosis in developing coping skills, coordinating services in the community, building relationships such as natural supports, and ensuring follow through with necessary mental health and medical appointments. The CM supports the client in understanding better what the barriers are, breaking them down, and supporting them in building resiliency so they can manage such relationships in the community without support. He alsomonitors progress and provides accurate and efficient documentation to ensure the clients needs are reflected.

Additional Tasks:

• Assess, plan, implement, monitor, and evaluate client’s health and needs
• Handle case assignments clients according to HOM Policy
• Create and implement treatment plans
• Help clients make informed decisions and advocate for client
• Provide education and support to client and client’s family
• Gather and make appropriate referrals to client
• Prepare and maintain client records and notes in data EHR
• Attend necessary trainings and certification courses
• Ability to travel locally
• Other duties as assigned
