Description

The Opportunity: Case Manager
Job Type: Full-Time, Hybrid
Pay: $55,000/year
What We Do

Through our powerful integrated behavioral health care model, we bring together vast resources from a diverse team of caring staff to help each client meet their personal goals. The care we provide paves the way for greater opportunity and second chances by focusing on the critical pillars towards a healthy life: behavioral health, financial stability, and safe housing.

As the Case Manager, you’ll have a dynamic and critical role in The Executive Office of Veteran Services (EOVS) program supporting the well‑being, stability, and self‑sufficiency of veterans and their families who are experiencing challenges or require additional support. The Case Manager is responsible for identifying and engaging veterans in need of services through outreach and collaboration with community partners, including community‑based organizations, hospitals, continuums of care, housing authorities, and emergency shelters. The Case Manager conducts comprehensive intakes and needs assessments, develops individualized service plans, and connects veterans to appropriate resources and supportive services. Through ongoing case management, advocacy, and coordination of care, the Case Manager helps veterans overcome barriers, access essential services, and achieve long‑term stability and independence.

Some of your critical responsibilities:

  • Conduct outreach and engagement with veterans in the community, collaborating with community‑based organizations, hospitals, continuums of care, housing authorities, emergency shelters, and other service providers to identify individuals and families in need of supportive services.
  • Complete client intakes, comprehensive needs assessments, and eligibility screenings to identify barriers to stability, determine service needs, and connect veterans to appropriate programs and resources.
  • Develop, implement, and regularly update individualized service plans with veterans, establishing measurable goals related to housing stability, employment, financial independence, healthcare, and other identified needs.
  • Provide ongoing case management services, including service coordination, referrals, advocacy, follow-up, and monitoring of client progress to ensure veterans are actively engaged in services and working toward established goals.
  • Collaborate with the larger VOAMASS veteran services team and external service providers to coordinate care, facilitate access to resources, prevent duplication of services, and ensure continuity of support for veterans and their families.
  • Assist veterans in navigating local, state, and federal benefit systems, including EOVS resources, VA benefits, housing assistance, employment programs, and other community‑based supportive services.
  • Establish and maintain strong working relationships with community partners, government agencies, and veteran service organizations to facilitate timely referrals, address service gaps, and advocate for the needs of veterans.
  • Maintain accurate, timely, and comprehensive client records, including assessments, individualized service plans, case notes, referrals, service outcomes, in accordance with VOAMASS policies, program requirements, and applicable state and federal regulations.
  • Track and monitor client progress, service utilization, and program outcomes to ensure compliance with grant requirements and performance expectations. Collect and submit required program data and monthly reports to support program evaluation and continuous quality improvement.
  • Participate in interdisciplinary team meetings, case consultations, and other program activities to identify client needs, address barriers to service delivery, and promote positive outcomes for veterans.

The Case Manager role is about making a difference in the lives of the people we serve. You’ll bring the following qualifications:

  • An Associate’s or Bachelor’s degree in Human Services, Social Work, Psychology, or a related field is preferred; however, a minimum of two years of relevant case management experience may be considered in lieu of a degree.
  • Knowledge of veteran‑specific challenges, homelessness, housing instability, and available local, state, and federal resources is highly preferred.
  • Established relationships with community‑based organizations, service providers, and employment resources are desirable.
  • Prior military service or personal experience navigating veteran support systems is preferred but not required.
  • Strong communication, organizational, and interpersonal skills, along with the ability to coordinate services and advocate effectively for veterans and their families are essential.

For full‑time employees, including our Case Manager, our comprehensive and generous benefits package includes:

  • Excellent health, vision, and dental insurance with 90% of medical deductible and certain co‑pays paid by employer
  • Multiple opportunities to use pre‑tax dollars to save for expenses such as health care (flexible spending account), child care, transit, and parking
  • 403(b) retirement plan with employer match
  • 24/7 nurse hotline for urgent health questions
  • Employee assistance program to provide help with family concerns, stress management, legal issues, and more
  • Employer‑paid long‑term disability and life insurance
  • Pet insurance
  • Tuition remission programs
  • Loan forgiveness (Public Service Loan Forgiveness (PSLF) program)
  • Generous vacation benefits, starting at three weeks and moving to four weeks after three years of employment
  • 14 paid holidays, higher than industry average
  • Fitness/wellness reimbursement to cover expenses such as gym membership
  • Employee discount program on hundreds of items such as computers, cell phone plans and accessories, furniture, hotels, vacation home rentals, etc.
  • Cash bonus, with no annual cap, for referring other qualified candidates for employment
