Job summary

Community Bridges, Inc. (CBI) is seeking a Case Manager to provide case management services in community and/or office settings across Arizona. The Case Manager supports patients and families by planning, coordinating, outreaching, advocating, and monitoring needs, and helps access health services, financial assistance, employment, education and social supports.

Embraces recovery and a safe environment. Ideal candidates have a high school diploma or GED, 1–3 years GMH/SU experience,

Qualifications

  • High school diploma or GED required.
  • 1-3 years of GMH/SU experience required.
  • 1-3 years in behavioral health position with outreach/engagement activities required.
  • 1-3 years of experience in a medical setting preferred.
  • Minimum 6 months of recovery from substance use and/or mental health disorders preferred.
  • BHT certification per A.A.C. R9-10-101.33 and CBI Policies.

Responsibilities

  • Provide case management in community and/or office settings.
  • Assist patients in gaining access to health services, financial assistance, employment, education and social supports.
  • Develop and monitor individualized treatment plans and document services timely.
  • Coordinate and advocate for patients' needs and progress.
  • Promote a recovery-focused, safe environment for clients.

CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.

Job Summary

The Case Manager provides case management services in either the community and/or in office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient’s needs and progress. The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services and natural supports. The Case Manager assesses patient needs, develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner. Embraces the recovery philosophy and promotes a safe recovery environment for clients.

Skills/Requirements
  • Highschool diploma or GED required. Associates degree of higher in a field related to behavioral health preferred.
  • 1-3 years of experience working with the GMH/SU population required.
  • 1-3 years of experience in behavioral health position providing outreach and engagement activities required.
  • 1-3 years of experience in a medical setting preferred.
  • Minimum of 6 months of recovery from substance use and/or mental health disorders preferred.
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures.
  • Valid Identification required.
  • Current AZ Driver’s License (valid and in good standing) preferred.
  • Clean 39-month driving record preferred
  • Arizona Fingerprint Clearance Card.
