Job summary

Family Promise Montco PA seeks a dedicated Case Manager in the Greater Philadelphia area to provide on-site support for families across emergency shelter, transitional housing, and permanent housing programs.

You will conduct intake assessments, develop individualized service plans, and coordinate housing and supportive services while coaching families in budgeting, employment readiness, health, and parenting.

Qualifications

  • Experience with trauma-informed care, cultural humility, and strengths-based approaches is highly beneficial.
  • Bachelor’s degree in social work or related field, or equivalent combination of education and relevant experience.

Company Description

Family Promise Montco PA, formerly Inter-Faith Housing Alliance, is a 501(c)(3) non-profit organization that has supported families experiencing homelessness and food insecurity in Montgomery County, PA for more than 40 years. The organization provides housing, food, and intensive case management to help families move toward long-term stability and independence. Services include education in life skills such as budgeting, job readiness, health, parenting, and more, tailored to each household’s needs. Programs span an emergency shelter in partnership with local congregations, a two-year transitional housing program, permanent supportive housing, and a community food pantry. Family Promise Montco PA also engages volunteers and community partners to strengthen support for families across the Greater Philadelphia area.

Role Description

The Case Manager is a full-time, on-site role based in the Greater Philadelphia area, primarily serving families in Montgomery County, PA. This position provides direct case management services, including conducting intake assessments, developing individualized service plans, and coordinating housing and supportive services across emergency shelter, transitional housing, and permanent supportive housing programs. The Case Manager will meet regularly with participants to monitor progress, provide coaching on life skills such as budgeting, employment readiness, health, and parenting, and connect families with internal and external resources. Day-to-day responsibilities include maintaining accurate case records, tracking outcomes, collaborating with staff, volunteers, and community partners, and advocating for participants within social service and housing systems. The role also involves supporting program operations at Ambler & Norristown properties, while contributing to a respectful, trauma-informed, and inclusive environment.

  • Experience working with families facing housing insecurity, poverty, or other barriers to stability is preferred.
