Job summary

Merakey is seeking a full-time Case Manager for the Dual Diagnosis Treatment Team in Penn Hills, PA. The role involves assessing and planning recovery-focused treatment and delivering therapeutic services to adults and youth with mental health diagnoses or autism spectrum disorders.

The position offers mileage reimbursement, a company car for travel, and room for growth within Merakey’s Adult Behavioral Health Program.

Qualifications

• Bachelor’s degree in social work, Psychology, or another human service-related field is required.
• Experience in the behavioral health/mental health field working with individuals with mental health/developmental disabilities is required.
• Valid PA driver’s license with a clean driving record and reliable transportation is required.

Responsibilities

• Assess, plan, and transfer recovery treatment and support plans for adults and youth with mental health diagnoses and/or autism spectrum disorders.
• Provide intensive therapeutic services in diverse settings, including community living arrangements, schools, and community settings.
• Collaborate with the treatment team to implement and transfer skills using a person-centered approach.

Position Details

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc. This candidate, in collaboration with other members of the treatment team, implements and transfers skills identified in the recovery treatment and support plan that utilizes a person-centered approach.
