Sga Youth seeks a dedicated Case Manager to provide comprehensive services for youth and families in violence prevention and survivor support programs. You will assess needs, develop service plans, and coordinate referrals with schools and community partners.
The role emphasizes advocacy, safety, and positive youth outcomes. Responsibilities include outreach, crisis intervention, caregiver engagement, and collaboration with behavioral health and social services to ensure coordinated care.
The Case Manager provides comprehensive case management services to youth and families participating in violence prevention, youth development, and survivor support programs. The Case Manager works collaboratively with schools, community partners, caregivers, and service providers to assess participant needs, develop individualized service plans, coordinate referrals, monitor participant progress, and ensure successful engagement in program services.
The Case Manager serves as a key advocate for participants by connecting them to educational, behavioral health, social service, and community resources that promote safety, stability, well‑being, and positive youth outcomes.
Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Criminal Justice, Education, or a related field.
Minimum of two years of experience providing case management, youth services, social services, or community-based support.
Knowledge of trauma-informed care, strengths-based practices, and youth development principles.
Strong interpersonal, organizational, and problem-solving skills.
Ability to maintain confidentiality and professional boundaries.
Proficiency with Microsoft Office and electronic case management systems.
Experience working with youth exposed to violence, trauma, behavioral health challenges, or justice system involvement.
