Job summary

CASES seeks a Care Manager to deliver person-centered care management, coordinating services and supporting members' health and well-being. You will develop individualized care plans, apply motivational interviewing, and engage families while prioritizing cultural sensitivity and recovery-oriented practices.

Position is full-time with flexible hybrid work options, primarily field-based, and 80% fieldwork requirement. Salary is $50,000 annually.

Qualifications

• High School diploma with CASAC certification or a relevant degree pathway.
• Broad educational routes that qualify for care management roles in NYC.
• Strong writing and communication skills essential for documentation.
• Proficiency with computers and health information databases.

Responsibilities

• Complete all documentation within required time frames in the portal.
• Provide core care management services and health promotion.
• Utilize motivational interviewing to support clients' strengths.
• Ensure access to needed services per the Care Plan (medical, housing, entitlements).
• Maintain minimum 4 monthly contacts per member, with 2 in person.
• Escort members to appointments; involve family in care planning.
• Respond to crisis situations as needed.
• Use HIT systems for reporting and data analysis.
• Stay updated on regulations and trainings.
• Deliver culturally sensitive, trauma-informed care.

Job Title: Care Manager

Job Summary

CASES is seeking a Care Manager that will provide person-centered care management services to empower individuals to take control of their health and well-being. You will develop and implement individualized care plans, leveraging motivational interviewing techniques and care coordination to support members in accessing necessary services. With a focus on cultural sensitivity, trauma-informed care, and recovery-oriented practices, you will provide guidance and support to members and their families. Shift Hours: This position is full-time Monday - Friday from 9:00 AM - 5:00 PM.

Salary: The salary for this role is $50,000 per year.

Location Address: 2090 Adam Clayton Powell Jr Boulevard, New York, NY 10027. Our office is easily accessible by public transportation.

Workplace Flexibility: Flexible - Staff have flexibility to complete their workday from home or another suitable space, typically after reporting to the office and/or engaging in field or community-based work. At least 80% fieldwork required.

What You Will Be Doing

• Complete all documentation - e.g., assessments, plans of care, encounters - within required time frames in the web-based health home portal.
• Provide core care management services (Comprehensive Care Management, Care Coordination & Health Promotion, Comprehensive Transitional Care, Enrollee & Family Support, Referral to Community & Social Supports, Use of Health Information Technology (HIT) to Link Services).
• Provide care management services using motivational interviewing techniques to support consumers to address their problems by utilizing their strengths and abilities.
• Assuring that member has access to engages in and retain needed services as defined in the member’s Care Plan; Services may include but not limited to: Acute, Primary, and Preventative Medical Care, Home Health Care, Chemical Dependency Services, Behavioral Health Services, Community social support, housing, state & federal entitlements.
• Meet required levels of case management contacts, a minimum of 4 per month per member, 2 of the 4 contacts being face to face with member in person.
• Assist members by escorting them to appointments, e.g., medical, behavioral health, entitlements.
• Involve family and significant others in the consumer’s care planning.
• Respond to crisis situations as needed.
• Use available databases and their reporting functions to conduct job duties, such as GSI Health Coordinator, ePACES/eMedNY, Health Commerce System Medicaid Analytics Performance Portal.
• Keep abreast of current city, state, and federal regulations, laws, and initiatives pertaining to health home care management and Medicaid redesign; attend relevant training.
• Provide all services in a culturally sensitive, trauma-informed, and recovery-oriented manner.
• Other duties as defined by the team leader or CASES executive staff.

What We Are Looking For

• High School Diploma and CASAC certified; or
• Bachelor's degree with 4 years of social services experience; or
• Bachelor’s degree and a NYS teacher’s certificate; or
• Bachelor’s degree and NYS licensure as a Registered Nurse; or
• Master's degree with 2 years of social services experience
• 2 years' experience in providing direct services
• Strong knowledge of wellness, recovery, and self-help.
• Effective communication and written skills are essential.
• Proficiency with computer and databases.

Preferred but not required

• Bilingual skills in Spanish highly preferred.
