Job summary

Randstad Enterprise partners with The Center for Urban Community Services (CUCS) to recruit dedicated professionals for The Hegeman in Brooklyn. This full-time, on-site role supports residents facing housing instability and complex needs.

Responsibilities include case management, service planning, benefits access, crisis intervention, fieldwork, and daily living support, with a trauma-informed, person-centered approach.

Qualifications

• Experience with trauma-informed, person-centered care is required.
• Proficiency in Motivational Interviewing is preferred.
• Experience with Supported Employment concepts is a plus.

Responsibilities

• Case management for a designated caseload on-site and in the community.
• Develop, implement, and update Individualized Service Plans (ISPs).
• Assist clients with public benefits (Medicaid, SNAP, SSI) and tenancy supports.
• Deliver crisis intervention and coordinate with emergency responders.

CUCS is one of the nation's largest and most acclaimed providers of housing and social services. The Hegeman is a LEED-certified permanent supportive housing residence in Brownsville, Brooklyn, providing a home to 160 low-income tenants, many of whom are overcoming histories of mental illness, homelessness, and substance abuse.

• Client: The Center for Urban Community Services (CUCS) / The Hegeman
• Employment Type: Full-Time
• Location: 39-47 Hegeman Avenue, Brooklyn, NY 11212 (On-site and community fieldwork)
• Schedule: Standard daytime hours (requires flexibility for crisis intervention and community escorts)
• Case Management: Provide comprehensive, strengths-based case management to a designated caseload of clients both on-site and throughout the community.
• Service Planning: Develop, implement, and continuously update Individualized Service Plans (ILPs) focused on housing stability, recovery, and overall wellness.
• Benefits & Housing Advocacy: Assist clients in securing and maintaining public benefits (Medicaid, SNAP, SSI) and support long-term tenancy preservation, including apartment inspections and annual recertifications.
• Crisis Intervention: Deliver effective crisis de-escalation, coordinate with emergency responders, and assist with hospital discharge planning.
• Fieldwork & Group Support: Accompany clients to essential medical, housing, or court appointments, and facilitate psychoeducational or recreational group programming.
• Daily Living Support: Assist with activities of daily living (ADLs), hoarding mitigation, and medication self-administration protocols.

What We Are Looking For

We are looking for candidates with a strong commitment to evidence-based practices like Motivational Interviewing and Supported Employment. The required experience scales based on your education:

• High School Diploma/equivalent: 2 to 8 years of relevant work experience (depending on tier).
• Bachelor's Degree / BSW: 1 to 4 years of relevant work experience.
