Status: Full Time, Regular, Non Exempt
Location: Rockaway Avenue, Brooklyn 11212
Salary: $27.48 to $30.22 per hour (approximately $50,000 to $55,000 annually)
Position Summary:
The Case Manager will screen, assess, counsel and link homeless, formerly incarcerated clients with histories of mental illness, substance use and/or HIV/AIDS to needed transitional and permanent housing.
Mission, Culture and Fit:
The candidate must have a history of working with a population who have been diagnosed with serious mental illness (SMI) and co-occurring disorders. In addition, the person must be able to interact and work with persons impacted by the legal system and have a history of homelessness. Demonstrated ability to work in a team environment.

Travel Requirements: This position requires weekly on-site availability and travel to various Fortune housing and program locations as required.
