Status: Full Time – Non Exempt
Salary Range: $27.47 - $30.22 per hour
Location: 625 West 140th Street
Days/Hours: Monday – Friday 9am - 5pm (Some flexibility required)

Position Summary:
The Case Manager will perform initial assessments, provide ongoing supportive services both in the field and in the office, and maintain documentation requirements for justice-involved clients with histories of mental illness, substance use, and homelessness.
Essential Duties and Responsibilities:
Qualifications:
Education: Associate’s Degree in Human Services or/and five years of experience working with special needs population; a High School Diploma and eight years of experience working with special population or a bachelor’s degree in human services is preferred with at least 2 years of experience with the target population.
Work Experience: Candidate must have the ability to oversee a caseload of 15 clients. In addition, must be able to provide a collaborative assessment of needs, create and implement service plan, monitor progress and evaluate services provided to the individual client.
Computer Skills: Proficient in Microsoft Excel, Word, and Outlook with a working knowledge of PowerPoint.
Communication Skills: Candidate will possess the ability to have excellent writing, editing and verbal communication skills.
General Work Behaviors/Skills: Excellent interpersonal skills with ability to interact culturally, linguistically, and diplomatically with diverse internal and external individuals.

Travel Requirements: Physical ability to travel throughout the 5 boroughs via public transportation in accordance with client location(s).
