The Case Manager is responsible for coordinating care and service delivery for an assigned group of patients, promoting communication and collaboration with patients, caregivers, physicians, and other health care team members. They assess, implement, coordinate, monitor, and evaluate services and resources to promote quality, cost-effective outcomes tailored to patient needs and resources. A Brief Overview

The Case Manager coordinates care and services for patients, fostering communication and collaboration within the health care team. They evaluate, plan, and monitor services to ensure quality and efficiency, including discharge planning and utilization review, while aligning with MLH's mission and values.

What you will do
  • Support a collaborative case management team to facilitate quality patient care, integrating service delivery, authorization, access, discharge, post-discharge activities, and utilization review.
  • Identify patients eligible for case management through appropriate screening and assessment.
  • Communicate with stakeholders and educate patients and families.
  • Plan effectively to meet patient needs, manage authorizations, and promote resource utilization.
  • Maintain systems to document, track, and monitor cases.
  • Perform other duties as assigned.
Education/Formal Training Requirements
  • Associate's Degree in Nursing (RN)
Work Experience Requirements
  • 1-3 years of clinical healthcare experience (acute or hospital)
Licenses and Certifications Requirements
  • Accredited Case Manager (ACM) - American Case Management Association
  • Case Manager - The Commission for Case Manager Certification
  • Registered Nurse licenses for Arkansas, Mississippi, and Tennessee
Knowledge, Skills, and Abilities
  • Excellent communication skills, oral and written.
  • Ability to collaborate effectively with physicians and health professionals.
  • Strong facilitation skills.
  • Effective planning and scheduling abilities.
  • Basic computer skills; advanced skills preferred.
  • Independent judgment, critical thinking, initiative in decision-making.
Supervision Provided by this Position
  • No supervisory responsibilities; may delegate to UR Specialists and Social Workers.
Physical Demands
  • Activities include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motions.
  • Must have good balance and coordination.
  • Light work exerting up to 25 lbs. occasionally and 10 lbs. frequently.
  • Requires close visual acuity for data analysis, reading, and computer work.
  • Frequent non-invasive patient contact.
  • Work environment similar to general office or administrative settings.
  • May include weekend and holiday rotations.
