Job summary

South Shore Association dba SAIL seeks a qualified counselor to support residents in an Apartment Treatment Program. You will provide training in independent living skills and supportive counseling to promote psychiatric stability and independence.

You will coordinate admissions and discharges, conduct weekly field services, and work with residents to develop personalized goals and plans while connecting them to clinical, vocational and community resources.

Qualifications

  • High School Diploma plus 5 years' experience in Human Services or an Associate's Degree plus 3 years in Human services.
  • Bachelor's degree in Human services related field.
  • Critical thinking and sound judgment skills.
  • Ability to manage change in a fast-paced environment.
  • Demonstrates organizational and communication skills for interaction with staff, clients, families and the community.

Responsibilities

  • Teach residents independent living, tenancy, housekeeping, socialization and community-integration skills.
  • Coordinate resident admissions, transfers, and discharges with weekly field services and apartment meetings.
  • Develop, evaluate and update person-centered service plans and goals with residents.
  • Provide supportive counseling, crisis intervention, case management, referrals to resources.
  • Support medication self-administration, explain effects and emergency procedures.
  • Promote safe housing by inspections, repairs, fire safety and disaster preparedness.

General Description

Responsible for providing supportive counseling and providing training in independent living skills to residents in the Apartment Treatment Program.

Duties and Responsibilities
  • Teach residents independent living, tenancy, housekeeping, socialization, vocational, educational, and community-integration skills that support independence and psychiatric stability.
  • Coordinate resident admissions, transfers, and discharges, and provide weekly face-to-face field services and regular apartment meetings.
  • Partner with residents to develop, evaluate, and update person-centered service plans and strength-based individualized goals.
  • Provide supportive counseling, crisis intervention, case management, collateral contacts, and referrals or linkages to clinical, vocational, educational, recreational, and community resources.
  • Support medication self-administration, teach medication effects and emergency procedures, and promptly notify supervisors of medical, psychiatric, or programmatic emergencies.
  • Promote safe, well-maintained housing by inspecting apartments, coordinating approved repairs, monitoring fire-safety equipment, and teaching fire safety and disaster preparedness.
  • Provide transportation and distribute resident funds as authorized; coordinate treatment-team activities; complete documentation promptly; attend required meetings and training; and follow agency policies and other assigned duties.
Schedules
  • Training Period: Mondays-Fridays 9am-5pm
  • Tuesdays-Saturdays
  • Sundays-Thursdays
  • 1 evening shift minimum/week (ending at 6/7pm)
