Job summary

South Shore Association dba SAIL is seeking a dedicated counselor to support residents in an Apartment Treatment Program. The role focuses on practicing independent living skills, tenancy, and community integration to foster psychiatric stability.

You will coordinate admissions and discharges, provide crisis intervention, and collaborate with residents to set person-centered goals. Located in Farmingdale, NY with field work in Suffolk County.

Qualifications

Responsibilities

• Provide supportive counseling and training in independent living skills.
• Teach residents independent living, tenancy, housekeeping, socialization, and community-integration skills.
• Coordinate admissions, transfers, and discharges; conduct weekly field services and apartment meetings.
• Develop person-centered service plans and goals with residents.
• Offer counseling, crisis intervention, case management, and referrals to resources.
• Promote safety and manage housing conditions, fire safety, and safety training.
• Assist with transportation, manage resident funds, and document promptly.

Qualification and Competencies

• High School Diploma plus five years' experience in the Human Services Field OR An Associate's Degree in Human Services plus three years of experience in the Human services field OR Bachelor's degree in Human service related field
• Critical thinking and sound judgment skills are necessary.
• Ability to manage change in a fast-paced growing business environment.
• Computer/office equipment usage
• Ability to work independently and as part of a team.
• Demonstrates organizational and communication skills which would enable effective interaction with agency staff, consumers/clients/families, and the community.

General Description

Responsible for providing supportive counseling and providing training in independent living skills to residents in the Apartment Treatment Program.

Duties and Responsibilities

• Teach residents independent living, tenancy, housekeeping, socialization, vocational, educational, and community-integration skills that support independence and psychiatric stability.
• Coordinate resident admissions, transfers, and discharges, and provide weekly face-to-face field services and regular apartment meetings.
• Partner with residents to develop, evaluate, and update person-centered service plans and strength-based individualized goals.
• Provide supportive counseling, crisis intervention, case management, collateral contacts, and referrals or linkages to clinical, vocational, educational, recreational, and community resources.
• Support medication self-administration, teach medication effects and emergency procedures, and promptly notify supervisors of medical, psychiatric, or programmatic emergencies.
• Promote safe, well-maintained housing by inspecting apartments, coordinating approved repairs, monitoring fire-safety equipment, and teaching fire safety and disaster preparedness.
• Provide transportation and distribute resident funds as authorized; coordinate treatment-team activities; complete documentation promptly; attend required meetings and training; and follow agency policies and other assigned duties.

Schedules

• Training Period: Mondays-Fridays 9am-5pm
• Tuesdays-Saturdays
• Sundays-Thursdays
• 1 evening shift minimum/week (ending at 6/7pm)
