Summary
The Case Manager will work in our Transitional Independent Living Program (TIL) and be responsible for ensuring client safety and providing care coordination and case management services in an effective and efficient manner for all youth ages 16-20 living in the facility.
Work Schedule
Full time, Monday - Friday, 10:00 a.m. - 6:00 p.m.
Responsibilities
- Manage and counsel clients in the Transitional Living Program. Assist clients in developing independent living skills through individual and group sessions.
- Identify immediate needs and connect clients to appropriate services. Monitor engagement and accompany clients to appointments as needed.
- Provide crisis intervention and stabilization services.
- Coordinate ongoing care related to housing, savings, medical, employment, education, mental health, and substance abuse. This includes making referrals, monitoring service use, case conferencing, and follow-up.
- Participate in weekly case conferences with the staff psychiatrist, vocational counselor, and social worker regarding youth progress and needs.
- Lead weekly groups on topics such as ADL skills, budgeting, employment, health awareness, and housing readiness.
- Maintain accurate documentation including intakes, service plans, progress notes, savings forms, and medical records, ensuring timely updates.
- Attend scheduled supervision and team meetings.
Qualifications Needed
- Bachelor's degree (BA/BS) required.
- One to two years of experience in case management, crisis intervention, or social services.
- Knowledge of housing services and entitlements.
- Valid driver's license and clean driving record.
- Excellent oral and written communication skills, especially regarding complex and sensitive issues.
