Job summary

African American Planning Commission Inc. in New York City seeks a Case Manager Supervisor to guide a team of caseworkers, ensure timely intake processing, and maintain policy compliance. You will review intakes, support residents, and coordinate resources with health, mental health, and housing services.

The role requires a Bachelor’s degree and 1–2 years of supervision; strong communication and data-driven practice are essential. Base salary and benefits are provided.

Reporting Relationships: The Case Manager Supervisor reports to the Director of Social ServicesClinical Services and Program Deliverables:Active participant in the guidance, support, development, and intervention of assigned caseworkers.Create caseloads and related duties. Review intake packet assigned to CM to complete.Actively participates in improving processes, procedures, policies, and assists in ensuring case manager’s compliance.Review intakes to ensure compliance of documents or timely processing of all required forms and processes.Ensure that the client's initial social service assessment is completed within 48 hours of their arrival.Ensure case managers obtain information including but not limited to medical, assessment referrals psychological, and social factors contributing to the situation. And that the staff is evaluating the resident's capabilities.Regularly inspects case records to evaluate the completeness and quality of independent living plans.Review appropriate referrals are generated and received by the residents as a result of incidents that occur onsite related to Substance Use, Mental Health, or Medical in nature.Participate in resident’s case conference and perform incident report follow-ups, as directed.Educate personnel on the various clinical services, recreational activities, and AAPCI programs that are offered, as well as community resources.Support caseworkers by providing additional resources for professional development and client services.May be required to complete supervisory review notes when reviewing case files.May be required to conduct initial assessments, biopsychosocial assessments, and ILPs.Required to carry a caseload as directed.Perform other related duties as may be requested.Assists in Directing and Supervising Personnel:Ensures caseworker’s compliance with the agency's time and attendance policy; recommends disciplinary action to supervisor for those out of policy.Assists the Social Services Director in staff performance management, including completion of performance reviews and counseling or disciplinary memos.Conducts staff supervision in accordance with program and division policy.Ensures that caseworkers have attended required in-service training and schedules employees to attend both in-service training and training provided by vendors.Supervision and Professional Development:Required to meet with Director of Social Services and/or Program Director for regular schedule supervisions to improve department goals and objectives.Attend all training issued by DHS and AAPCI to improve the delivery of services.Attend external training as scheduled by the Director of Social Services or Program Director.Participate and assist with facilitating social service meetings as directed.Other development training as assigned.Minimal Qualifications:Degree Requirement: Bachelor’s Degree Required in a relevant field (Psychology, Sociology, Education, Counseling, Substance Use, Mental Health Counselor, Social Worker, Coaching, Leadership, etc.)At least 1-2 years of Full-Time experience in a case management-related position.Experience with supervising a team of 2-6 members requiredOther Qualifications:Experience working with the NYC Department of Homeless Services and DHS CARES system is preferred.Ability to work collaboratively with the Program Director, Director of Social Services, social service team, and operations to deliver impactful service for shelter residentsStrong verbal and interpersonal communication skills with a focus on providing excellent client services;Demonstrated ability to interact effectively and collaboratively with a diverse community of residents, program staff, and external partnersStrong time management skills and organization skills.High attention to detail.Physical Requirements:• Prolonged periods of sitting at a desk and working on a computer• Must be able to lift to 5 pounds at times• Must be able to access and navigate each department at the organization’s facilities• Strong understanding of the NYC homeless shelter system• Prolonged periods sitting at a desk and working on a computerCompensation and BenefitsBase salary range and benefits information for this position are being included in accordance with requirements of New York State and City pay transparency laws.
